Malformed/edge-case tool calls that errored during tool lookup or schema validation (outside the inner handler guard) escaped uncaught and produced no JSON-RPC response, hanging live stdio clients. mcp-server-handle-message now wraps the whole request branch so any error returns a JSON-RPC error response. Fixes the folio note-patch hang and the fjo tools/list crash.
